چکیده

RRadiofrequency catheter ablation is a well established modality of treatment for ventricular tachycardia (VT). But it may not be successful in certain cases when the re-entrant circuit is situated deep within the myocardium and inaccessible to both endocardial and epicardial ablation. One of the earliest reports on transcoronary ethanol ablation of refractory ventricular tachycardia was by Inoue H et al in 1987 [1] in experimental animals followed by Brugada P et al in humans in 1989 [2]. Brugada P et al identified the artery supplying the arrhythmogenic area and injected 1.5-6ml of sterile 96% ethanol, which cured the arrhythmia in two of the three patients. In the third one, arrhythmia recurred when a new collateral blood supply developed to the arrhythmogenic region, which was successfully treated by a repeat procedure. They had done a careful transcoronary mapping with saline prior to ablation in all cases. One of their patients developed temporary complete atrioventricular block after high septal ethanol ablation, requiring pacemaker.

Transcoronary Chemical Ablation of Ventricular Tachycardia

After identification of the artery supplying blood to the arrhythmogenic area, transcoronary chemical ablation of ventricular tachycardia was undertaken in three patients with incessant tachycardia in whom the other therapeutic options had failed. Sterile ethanol (96%) was given at a dose of 1.5 ml in two patients and a total of 6 ml in the third. The arrhythmia was cured in two patients and su...

Transcoronary chemical ablation of ventricular tachycardia.

Transcoronary chemical ablation of ventricular tachycardia was recently performed in one patient with refractory ventricular tachycardia in our hospital. The procedure was successful. The arrhythmia was cured and no recurrence occurred in a 24-month period. The clinical application and mechanism of transcoronary chemical ablation were discussed.

Role of alternative interventional procedures when endo- and epicardial catheter ablation attempts for ventricular arrhythmias fail.

BACKGROUND Ventricular tachycardia (VT) refractory to antiarrhythmic drugs and standard percutaneous catheter ablation techniques portends a poor prognosis. We characterized the reasons for ablation failure and describe alternative interventional procedures in this high-risk group. METHODS AND RESULTS Sixty-seven patients with VT refractory to 4±2 antiarrhythmic drugs and 2±1 previous endocar...

Transcoronary ethanol ablation for recurrent ventricular tachycardia after failed catheter ablation: an update.

BACKGROUND Despite substantial progress, radiofrequency catheter ablation (RFCA) fails in some patients. After encouraging results with transcoronary ethanol ablation (TCEA), we began offering TCEA routinely when endocardial and epicardial RFCA failed or a deep intramural substrate was likely. METHODS AND RESULTS Among 274 consecutive patients who underwent 408 ventricular tachycardia (VT) ab...

Ventricular tachycardia arising from the aortomitral continuity in structural heart disease: characteristics and therapeutic considerations for an anatomically challenging area of origin.

BACKGROUND The aortomitral continuity (AMC) has been described as a site of origin for ventricular tachycardias (VT) in structurally normal hearts. There is a paucity of data on the contribution of this region to VTs in patients with structural heart disease. METHODS AND RESULTS Data from 550 consecutive patients undergoing catheter ablation for VT associated with structural heart disease wer...
